Not all care needs are ongoing. Sometimes, what’s really needed is the right support, at the right time, to help you get back on your feet.

The Restorative Care Pathway is a short-term program under Australia’s Support at Home system. It is designed to help older people regain or maintain independence after illness, injury, hospital stays or a gradual decline in function.

Rather than providing ongoing assistance, restorative care focuses on recovery, strength, confidence and everyday capability.

What is the restorative care pathway?

The restorative care pathway provides time-limited, goal-based support delivered by clinical and allied health professionals. The aim is simple – to help people do more for themselves again.

This might include improving mobility after a fall, rebuilding strength following hospitalisation, or learning safer ways to manage daily activities at home.

Care is tailored to the individual, with clear goals agreed between the person, their family and their provider.

Who is restorative care designed for?

Restorative care may be suitable if you or a loved one:

  • Have recently been in hospital or experienced illness or injury
  • Have had a fall or noticed changes in balance or mobility
  • Are finding everyday tasks harder than before
  • Want to avoid long-term care if possible
  • Would benefit from short-term allied health or nursing support

Eligibility is determined through an aged care assessment. People such as hospital discharge planners, social workers and other health staff assisting people to access the pathway must apply through My Aged Care. If restorative care is appropriate, it will be approved as a standalone funding pathway under the Support at Home program.

How restorative care works

Once approved, restorative care is delivered over a short, defined period – usually up to 16 weeks.

At the start, your provider works with you to develop a care plan. This outlines what you want to achieve, how support will be delivered and how progress will be measured.

Services are focused and designed to build confidence and independence, rather than create reliance.

Restorative care can be delivered in your home, in community settings, or a combination of both.

What services can be included?

Restorative care typically includes clinical and allied health services such as:

  • Physiotherapy
  • Occupational therapy
  • Nursing care
  • Exercise and mobility programs
  • Falls prevention strategies
  • Support with daily living skills

All services are selected to support your specific goals, not as a standard package.

How restorative care is funded

Restorative care is government-funded as a standalone funding pathway under the Support at Home program.

Clinical and allied health services provided through the pathway do not attract a co-contribution. This makes restorative care an accessible option for people who need short-term clinical support.

In some cases, additional funding may be approved if further services are needed to help achieve agreed goals.

What happens at the end of restorative care?

Restorative care is designed to end once goals are met or the approved period finishes.

Towards the end of the program, your provider prepares an exit plan. This may include:

  • No further services if independence has been restored
  • Help with transitioning to standard Support at Home services
  • Referral to longer-term care if required

The focus is always on the least restrictive, most independent outcome possible.
