How it works

Start with your ACAT assessment
Visit My Aged Care or phone 1800 200 422 to request an assessment. Once you’ve been assessed and know your Support At Home level, we’ll use your assessment to build a care plan that makes the most of your funding.

Get to know your local Envigor care team
Our experienced Care Services Manager and Care Co-ordinators will pair you with friendly qualified nurses in your area. If you’d like to request a specific nurse, talk to us – we’ll do our best to make it happen.
3.
Care delivery and monthly clinical reviews
Your mix of clinical, everyday and lifestyle support services commence. We complete monthly clinical reviews to make sure your care is working for you. Care plans are flexible and updated as your needs change.

Frequently asked questions
Is nursing care included in my Support at Home funding?
Yes. Nursing is part of the clinical support category and is fully funded under Support at Home. If you have been assessed as requiring clinicalsupport, nursing care is funded.
How often can I have a nurse visit?
It depends on your health needs and funding level. We’ll work with you and your care coordinator to find the right schedule.
Do I need a GP referral for nursing care?
No referral is required for funded clients, but we’ll often collaborate with your GP or specialist to ensure continuity of care.
Can nurses help after a hospital stay?
Yes. We regularly assist with discharge and recovery — including wound care, medication support and health monitoring.
Will I see the same nurse each time?
We do our best to ensure consistency, so you can build a comfortable, ongoing relationship with your nurse.
How often is my nursing care reviewed?
You don’t need to wait for a scheduled review. Just contact your care coordinator and we’ll arrange for your nurse to visit sooner or adjust your plan.
What if my health changes between reviews?
Our nurses review your care plan every month, ensuring you get the exact care you need to stay healthy and safe at home.
Are your nurses employed by Envigor or contracted?
Our nurses are employed by Envigor. They’re qualified, insured and part of our permanent local teams across Queensland.
Do your nurses work weekends or after hours?
Yes. Nursing care can be arranged on weekends, public holidays or after hours when needed, especially for clinical or post-hospital support.
