Can you pick me up from hospital?
Yes. We can arrange transport from hospital to home, and we’ll have the right staff waiting when you arrive to ensure your return home is safe.
How soon can my post-hospital care start?
If we’re contacted while you’re still in hospital, we can have your care plan ready for discharge day. We’ll be there when you need us, and can provide your transport home from hospital.
Is post-hospital care covered by Support at Home funding?
Yes. Post-hospital care is covered under clinical and personal support categories. If you don’t yet have a package, we can provide services privately until your funding is approved.
Can you help with wound care and medication?
Absolutely. Our registered nurses can manage dressings, monitor wounds and assist with medication and recovery routines.
Can family be involved in my care plan?
Yes. We encourage families to be part of your recovery plan and keep them updated on your progress.
Do you coordinate with hospital discharge planners?
Yes. Our team regularly works with discharge planners, social workers and allied health staff to ensure care continues seamlessly once you leave hospital.
