We recognise that people are increasingly living with chronic conditions that, if not managed appropriately can lead to long term disability. To better support those living with a chronic disease to manage the effects of their condition, Envigor has partnered with GRIT Health to provide a chronic disease monitoring service to Envigor clients.
Supported by a team of highly skilled health professionals and case managers, users of the GRIT Health services have access to their own personalised health support team, through the app on their smart phone or device.
If a person using GRIT Health becomes unwell, the monitoring team will know about and they will do something about it.
Through regular monitoring of the vital signs and symptoms of the condition, people with a chronic disease will have a better understanding of their condition and will be better able to manage the effects of this condition.
